What is Fetal Health Surveillance

FHS for short

Focuses on reading and understanding fetal (or baby) heart rate during labour. Determines the baby’s well-being during labour.

Based on guidelines from the SOGC (Dore & Ehman, 2020).

Not everyone gets a FHR (or fetal heart rate) tracing during labour.

 

Low-risk birthing people need the baby’s heart rate listened to periodically with a Doppler.

Birthing people with risk factors needs a FHR tracing while in labour.

Fetal Heart Rate

What is normal?

110-160

Has a baseline heart rate

Focus on the baseline heart rate

·        Similar to adults ‘resting heart rate’

Accelerations

·        Focus on a spike in heart rate above the baseline

A spike in Fetal Heart Rate above the baseline is an Acceleration. This usually corresponds to the baby’s movement.

Decelerations

·        Focus on the dips in the heart rate below the baseline

A drop in the fetal heart rate below the baseline is a deceleration. There are multiple types of decelerations, some of which are normal and others not.

The types of decelerations are: Early, Late, Variables, and Prolonged.

We will not go into detail about the types of decelerations at this time.
